Mortality and Functional Recovery after Bipolar Hemiarthroplasty of Femoral Neck Fractures in Elderly Patients

Abstract
|

|
Purpose: We evaluated the usefulness of bipolar hemiarthroplasty after femoral neck fracture in elderly patients by analyzing the mortality and fuctional recovery during the postoperative period.
Materials and Method: We retrospectively studied 96 patients, ranging from 65 to 102 years old(average 78.8), with femoral neck fracture who had been treated by bipolar hemiarthroplasty from March, 1990 ro March, 2000. The postoperative mortality, cause of death, correlation with age, sex, preoperative general condition, and time to surgery(Ed-i.e. only correlated iwth respect to ’age’. However, if the correlation was investigated with respect to the other three conditions also, then insert "and". before ’correlation’)were evaluated. Subjective satisfaction score, Harris hip score, ability to walk, and activity of daily living were assessed for the surviving patients.
Results: The overall mortality rate was 37.5%, being 16.7% in the first year. Among the first year mortality cases, the operation related mortality rate was 8.3%. Suvival time of the dead ranged from 3 weeks to 7 years 9 months(average, 2
years 8 months). First year mortality was affected by preoperative general condition(ASA grade) and timeto surgery. Excellent or good results were obtained by 26 patients(71.6%) in the subjective satisfaction score. Twenty-five patients(68.3%) regained the preinjury ability to walk, and 16(43.3%) the basic activity of daily living.
Conclusion: Even in elderly patients with femoral neck fracture, bipolar hemiarthroplasty is recommended for the relief of pain and functional recovery.
